El Sol Neighborhood Educational Center is committed to improving population health by implementing Community Health Worker (CHW) programs and interventions that address social determinants of health, enhance healthcare access, and promote culturally responsive health education for underserved communities.
By training and deploying CHWs using a population health promotion approach, we foster community engagement, improve health literacy, and facilitate connections to essential services. This ensures that diverse populations — including residents with limited English proficiency and those in low-income communities — receive the support needed for healthier, more connected lives.
